Okay, so Cole was human briefly in Season 4, and then became a demon again in Season 4, and was the Source. There were some signs Phoebe should've seen that he was a demon again and/or that he was now the Source. 1. In the Three Faces of Phoebe, she says he's avoiding her again, like he was when he a demon. 2. In The Three Faces of Phoebe, he conjures up a knife to stab a demon, then gives it to the girls, saying the demon left it and he stabbed the demon. 3. At the end of The Three Faces of Phoebe, Phoebe is suspicious enough about Cole that she asks him if there's something he's hiding from her or not telling her. 4. In Charmed and Dangerous, after Cole jumps in front of the Source to save Phoebe, the sisters see the Hollow leaving him. 5. In the Fifth Halliwell, Paige sees Cole using demonic powers on their innocent. After Phoebe leaves, Paige tries to tell Piper what she saw, but Piper brushes it off, saying Cole is human, he's not demonic, etc. 6. In the Fifth Halliwell, Cole shows up in a swanky sports car and gives Phoebe flowers and gives Piper and Leo the honeymoon they didn't get when they were first married. Who gets a sports car from their bosses on their day at a law firm and makes enough to send their brother- and sister-in-law on a honeymoon? 7. In The Fifth Halliwell, Cole gave Phoebe chocolates that the Seer had put a potion in, to make her want sex if I remember correctly. Phoebe and Piper both had some of the chocolate, and Piper suddenly can't wait to go on her honeymoon. 8. As Paige pointed out, Cole couldn't function in the job she got him at her work, but he does really well when he's pretending to be a lawyer at a fancy pants lawyer's office.

Everyone should have noticed all the things you pointed out. And if you think about it, Phoebe hadn't even known Cole for two years at this point, and they weren't even dating for all of it. I think Phoebe thought she knew him a lot better than she really did. If this storyline had happened in season 5 instead of 4, I'd like to think she'd have picked up on more of the weirdness of his behavior.
I also think Prue's death had a lot to do with Cole getting away with things for so long. It's just like Leo told Piper: they wanted a normal, happy family, and so that was what they saw. I think Piper was in so much grief after Prue's death that she put her head in the sand as far as the rest of her family goes. She just wanted things to be okay, even to the point of being in denial.
And for Phoebe, I think in some ways she felt free after Prue died, as Prue was always protective and sometimes judgmental. I think Phoebe got obsessed with the idea that she could now do what she wanted, and so she dove into this role of being Cole's wife, something she might have held back on if her smarter, oldest sister was still around to make her think more about things.

If Phoebe had been written the way she was in season 2 and Prue was alive she definitely would've sensed something was up and questioned it then but those the dibs obviously. Phoebe would've also admitted the part she played in what happened with Cole season 5 in terms of her choosing him over her sisters and been Queen Of The Underworld which she chose herself before any of the Seers tonics was introduced naturally.
Unfortunately for those two, they weren't right for one another and brought the worst out of each other hence why they never should've gotten together of course.

Like you pointed out, Piper was pretty grief-stricken, and she really wanted a normal life. Cole being human would've added a sense of normalcy that she wouldn't get with him being a demon.
